Our Referral Process
Referring a patient to Total Wound Care is straightforward and designed to ensure a smooth transition to in-home care. We prioritize simplicity and efficiency to get your patients the care they need as quickly as possible.
Complete the Referral Form: Fill out our secure referral Form, providing essential information about the patient’s wound care needs and medical history. This form helps us ensure we have all the information needed to begin personalized care.

Patient Intake and Eligibility Verification: Once we receive the referral form, our intake team verifies insurance eligibility, checks for Medicare coverage, and contacts the patient or caregiver to discuss care details and answer any initial questions.

Initial Assessment and Care Plan Development: During the first in-home visit, our wound care specialist evaluates the patient’s wound, assesses their health status, and develops a personalized treatment plan. We consider all aspects of the patient’s care, including dressing needs, recommended therapies, and frequency of visits.

Ongoing Care and Monitoring: Our team provides regular visits to manage wound care, monitor healing progress, and make any necessary adjustments to the care plan. Throughout this process, we communicate regularly with you, providing detailed updates on the patient’s status and any relevant changes in treatment.

Discharge and Final Report: When the patient has healed or transitioned to a lower level of care, we complete a discharge assessment and provide a final report summarizing the patient’s wound status, treatment outcomes, and any recommendations for ongoing care.
